Presentation and description:
The objective of NoHarm: Intelligence for Patient Safety is to improve health services management appraising for patient safety by integrating cutting edge technological solutions for Primary Health Care.
The NoHarm software, developed by the executor, is a solution that combines technology and human resources to foster comprehensive care, assuring patient safety through multidisciplinary endeavors. The system connects to patients’ electronic records, focusing on prescriptions review, medicine interactions, tests with alterations, adverse events, chronic illnesses, apart from acting on treatment monitoring through SMS.
Through artificial intelligence, the platform optimizes medicine prescriptions, treatment monitoring and handling of health requirements, making patient referrals easier and more assertive.
Besides, using real time control panels, the software improves monitoring and management of the health units, for example, to identify cases of extreme frequency at the unit (patients with little connection or very frequent users) and for the management of queues and referrals.
The speed of specialized care for high-risk patients and improvements in regulation and APS performance in all the involved municipalities are among the expected results, with patients’ safety as one of the main areas of focus.
