epCertify with Hiperdia Care Pathway

Presentation and description:

The project epCertify com Linha de Cuidado Hiperdia (freely translated to epCertify with Hiperdia Care Pathway) is a technological solution that aims to strengthen the monitoring of chronic diseases, in particular, hypertension and diabetes. Using technology, it is possible to automatically correct and provide a certification of the digital register for citizens over 18 years old, as well as monitor the care for people with hypertension and diabetes. This registration is carried out by the Family Health Team (Equipe de Saúde da Família) and Community Health Agents (Agentes Comunitários de Saúde).

To this end, the participating municipalities will have their infrastructure enabled to receive this digital solution. Thus, health agents will be able to incorporate data from the Citizen’s Electronic Medical Record (Prontuário Eletrônico do Cidadão – PEC) into the data collected by health agents, allowing the updating of the automated registry and the monitoring of hypertension and diabetes.

In this manner, problems such as record duplication or inconsistency, which are detrimental to monitoring users of the Brazilian Unified Health System (SUS), can be considerably mitigated or resolved. Such corrections avoid incorrect reporting to the Brazilian Ministry of Health, which can directly impact the financing of Primary Health Care in these municipalities.

Additionally, a major challenge in the care of Diabetes Mellitus and Hypertension, chronic noncommunicable diseases (CNCDs), is the fragmentation of patient data, which hinders the continuity of care. Thus, the care for these diseases may greatly benefit from gathering essential information, which is possible due to technologies that allow better population understanding, risk stratification and more appropriate monitoring.

The project is aligned with and will contribute to the current guideline of the Brazilian Ministry of Health, the Linking and Follow-Up Component, or “Componente Vínculo e Acompanhamento,” focused on the registration’s assessment and the effective follow-up of patients​.

Beneficiary description:

The beneficiaries are the Municipal Health Departments from 10 municipalities in the Northeast region of Brazil.

The project is supported by Together for Health, a BNDES initiative, in partnership with private donors, with the goal of gathering resources to expand access to public healthcare for the population in the North and Northeast regions of Brazil.

The epHealth Institute is a non-profit organization created in 2022 as a consolidation of robust social impact initiatives that had been developed since 2011. The Institute focuses on public health in Brazil, starting with Primary Health Care and Community Health Agents as the foundational structure of the Brazilian Unified Health System (SUS). Its activities include 4,274 cities impacted by free digital solutions, and 80 municipalities where projects were carried out in partnership. These activities have already reached more than 6 million patients of SUS, and 50 million home visits, which were enabled by the solutions.

Umane is an independent, non-partisan and non-profit civil society organization that promotes initiatives in the field of public health with the aim of contributing to a more resolute Brazilian Unified Health System (SUS) and improving the quality of life of people living in Brazil. Umane operates through three programs: Comprehensive Care for Chronic Non-Communicable Diseases (DCNT), with initiatives to control risk factors, screening, expanding access to health and monitoring risk factors in Primary Health Care; Strengthening of Primary Health Care (APS) as the coordinator of care in SUS, through support for initiatives aimed at improving operations, team productivity, integration of services and the incorporation of new technologies into the health system; and the Women’s, Children’s and Adolescents’ Health program, funding programs that track and monitor unfavorable outcomes during pregnancy and health conditions of children and adolescents in the context of Chronic Non-Communicable Diseases and risk factors.
