Presentation and description:

The general objective of CARDIO is to transform the scenario of Chronic Non-Communicable Diseases (CNCDs) and clinical outcomes, reducing morbidity and mortality rates in 25 municipalities in the Sertão of Paraíba, nine in Ceará and one in Pará.

The initiative seeks to optimize available resources and improve urban health management in Primary Health Care through the expansion of the CARDIO4Cities approach, adapted to the needs and cultural specifications of these territories. This is a population health approach designed and validated by the Novartis Foundation together with several partners and city authorities around the globe. The acronym CARDIO represents the overall objectives of the initiative, which are:

Enhance the quality of Care: standardize and improve the management of cardiovascular risk, such as hypertension, in Primary Care;

Guarantee and expand Access to health services: Expand tracking and early diagnosis of cardiovascular risk factors, such as hypertension;

Reform and influence public policy: Support governments in the implementation of health strategies and reforms;

Harness Digital Data and technology: Promote data-based management, to modernize the collection and analysis of health data;

Foster Intersectoral collaboration: Create strategic partnerships to strengthen the sustainability of the model;

Ensure Orientation, local ownership and sustainability: Strengthen local governance to ensure continuity.

Beneficiary description:

The beneficiaries of this project are the Municipal Health Departments and all their professionals involved in Primary Health Care in the 35 municipalities covered.

Ceará (Acarape, Barreira, Caucaia, Guaiúba, Maracanaú, Maranguape, Pacatuba, Palmácia, Redenção), Paraíba (Água Branca, Belém do Brejo do Cruz, Bom Sucesso, Brejo do Cruz, Brejo dos Santos, Catingueira, Catolé do Rocha, Desterro, Imaculada, Jericó, Juru, Malta, Manaíra, Mato Grosso, Piancó, Princesa Isabel, Riacho dos Cavalos, Santa Luzia, Santa Teresinha, São Bento, São José de Princesa, São José do Brejo do Cruz, Serra Grande, Tavares, Teixeira).

The project is supported by Together for Health, a BNDES initiative, in partnership with private donors, with the goal of gathering resources to expand access to public healthcare for the population in the North and Northeast regions of Brazil.

BP – Beneficência Portuguesa de São Paulo is a non-profit institution that has been dedicated to healthcare for 164 years and is one of the largest in Latin America. Recognized by the Brazilian Ministry of Health as a hospital of excellence, it has two hospital units, including one with care by the Brazilian Unified Health System (SUS). It offers care in several specialties and serves more than 224,000 patients a year. Focused on high complexity, the institution stands out in Cardiology, Nephrology, Neurology, Oncology and Pediatrics. In addition, it has a Research Center that develops clinical studies and seeks technical excellence and national and international recognition. Recognized by the National Accreditation Organization and the Joint Commission International, BP seeks to optimize its resources and positively impact the health sector through social projects, adapting to the concept of being a Health Hub.

The National Bank of Economic and Social Development (BNDES) is a federal public company linked to the Ministry of Development, Industry, Trade and Services. It is the Federal Government’s main instrument for long-term financing and investment in all segments of the Brazilian economy, with the aim of improving the lives of generations, promoting economic, social and environmental development. It operates throughout the country, from its office in Rio de Janeiro (RJ), where its activities are concentrated, with official headquarters in Brasília (DF) and regional offices in São Paulo (SP) and Recife (PE).

The Novartis Foundation (NF) is a non-profit organization based in Basel Switzerland. For over 40 years, it has helped improve the health of low-income populations, initially supporting disease elimination in areas such as leprosy and malaria. Today, the Novartis Foundation tackles the burning health issues of our time: cardiovascular disease and health inequity. It’s population health approaches CARDIO4Cities and AI4HealthyCities widen the lens from a narrow focus on individual healthcare delivery to a panoramic vision of improving health in a population at large. The Foundation’s work brings together existing but disconnected data to help authorities understand the root causes of inequitable health outcomes and find the best ways and partners to remediate them through targeted, evidence-based interventions. This empower governments to transform their health systems from being reactive to being proactive, predictive, and preventative, and achieving health equity among the populations they serve.
